TMJ disorders can be caused due to many pathologies. From something as a simple as an impacted or malpositioned tooth to a post traumatic injury. Hence, treatments can also vary from holistic appliance therapy to comprehensive surgical replacement.

Again, diagnosis is the key. Fortunately, almost 90% of all internal derangement TMDs have a muscle origin. Surgery can be almost totally avoided. I categorically state that unless, there’s a fracture or fusion of the bones in the TMJ (ankylosis), avoid surgery.

  • Arthroscopy helps diagnose the condition of the TMD better but flushing the joint with sclerosing agents will only do more harm; as it doesn’t take care of the hyperactive muscle.
  • Total joint replacement surgeries may help with pain, but because of the complex muscle attachments and movements of the TMJ, the patient may not get as functional as they are supposed to be.
  • Other surgical procedures such as condylectomy or condylar shaving, etc., are all attempts to recreate the lost space within the joint so as to enable the smooth functioning of the TMJ. When actually a better muscle tonicity and re-positioning is required to recreate the space.
